When the Medical Records Leave You With More Questions

Medical records can be lengthy, disorganized, and filled with technical language. Important details may be buried across thousands of pages, while inconsistencies, missing records,

pre-existing conditions, and gaps in treatment can directly

affect your case.

Covenant Legal Nurse Consulting helps you:

  • Understand the medical timeline
  • Identify inconsistencies and conflicting documentation
  • Distinguish pre-existing conditions from new injuries
  • Evaluate causation and damages
  • Identify missing records and documentation gaps
  • Prepare for depositions, mediation, settlement, and trial
  • Recognize medical issues before they become surprises

You do not need more pages to review. You need a clear understanding of what the records reveal.

Medical Details That Changed the Understanding of the Case

Pressure Injury Defense Review

In a case involving more than 8,000 pages of medical records, the documentation revealed repeated refusals of repositioning, a specialty bed, sequential compression devices, and prescribed medication.

Those details provided important context regarding the care offered, the patient’s participation, and the factors that may have contributed to the outcome.

Personal Injury Fall

A detailed chronology, medical summary, and case fact sheet organized the claimant’s treatment history into a clear medical timeline. This allowed the legal team to understand the key issues more efficiently and use the findings when developing case strategy.

Expedited Pre-Deposition Review

Shortly before a scheduled deposition, counsel needed help understanding the medical issues raised in a demand letter. An expedited review identified the facts affecting causation and damages and provided counsel with the medical insight needed to prepare for settlement discussions.

Pressure Injury Evaluation

A review involving four pressure injuries identified gaps involving the specialty mattress, wound measurements, photographs, documentation, and follow-up care. The findings helped clarify both the documented care and the information that was missing from the medical record.
